I am trying to create a different shape to do box modeling from, but the top and bottom are missing when I use the extrude tool.

By default, the snap tools in max are set to snap to grid points only. To change these settings right click on one of the snap tool buttons and adjust the settings as you see fit.

As you can see in the image here, my spline has a curve which I setup with the bezier handles, but it appears as a bunch of segments, how can I make it nice and smooth?

You need to use interpolation to make the spline appear more smooth and curved.
